If you are enrolled in the Basic or Enhanced Medicare Rx Option and need to mail in a claim form for reimbursement, please note the new address: Prescription Solutions, P.O. Box 29046, Hot Springs, AR 71903.
You can get a copy of the paper claim form by downloading the HOP Prescription Drug Claim Form on the Forms page or by calling Prescription Solutions at 1-888-239-1301 (TTY/TDD: 1-800-498-5428).
