Dosage form indicates the dosage amount covered (e.g., 10mg, 1mg, and 2.5 mg tablets).

Status indicates the type of drug.
- Tier 1 - Generic
- Tier 2 - Brand, which is a brand-name drug with no generic equivalent
- Tier 3 - Multi-source brand, which is a band-name drug that has a generic equivalent
- Tier 4 - Specialty Drug
